2. Clear photocopy of your Credit Card (front side only)
3. Copy of any one of the Photo ID proofs such as Passport, Permanent Account Number (PAN) Card, Voter ID Card, Driving License, Government ID Card, Photo Ration Card and Senior Citizen ID Card
4. Proof of presence, such as:
a) HR letter confirmation, in case you were present in the office on the date of the disputed transaction(s). The letter should be on the company letter head, duly signed and stamped.
b) Statement of usage of any other bank's card on the date of the disputed transaction(s) confirming your availability in that particular place and city.
c) Hotel stay proof on the date of the disputed transaction(s) confirming your presence in that particular place and city
d) Itemised post-paid mobile bill of the cardholder for the period when the disputed transaction(s) took place.
5. If you were not present in the country when the disputed transaction(s) took place on Mon DD, YYYY, please provide a copy of all the pages of your passport. In case of a joint account, please provide a copy of all the pages of the passports of all account holders
6. A copy of the police complaint, if lodged.
We will review the documents received from you and will contact you in case we need any further clarification. If the details are adequate for a resolution, we will complete the investigation.
Please note that reversal of the amount will be subject to the investigation. A correspondence to the same effect will be sent to you on your registered e-mail ID.
