Important Communication regarding your Demat DIS Booklet for your Demat Account IN302679 XXXXXX 48

We write with reference to the SEBI Circular No. CIR/MIRSD/01/2014 dated January 7, 2014 and subsequent Circular No.CIR/MRD/DP/22/2014 dated July 4, 2014 regarding the Standardisation, Monitoring and Scanning of Delivery Instruction Slip (DIS) for Demat Accounts. As per the provisions of the above mentioned circular, the DIS booklet issued prior to October 1, 2014 have been rendered invalid for further use. As per our records, there is a DIS booklet issued to your account which is in 'Unused' status. We request you to not use the referred booklet for Demat Transactions. Please destroy the same at your end for security reasons. You may opt for issuance of the new DIS booklet by: Logging into internet banking and making a request for the new DIS Booklet. Calling our Customer Care or writing to us at customer.care@icicibank.com through your registered e-mail address. Submitting the attached Booklet Request Form, duly filled and signed by all holders at the nearest ICICI Bank Demat Servicing Branch. Please carry the original proof of identity document for verification. There will be a charge of ₹50 per booklet containing 10 leaflets plus applicable taxes.
